New Client Form - For first time clients.

Financial Policy - For all clients. To be filled out one time, no matter how many pets, by each client. The Animal Clinic of West Plains requires payment in full at the end of your pet or horse's examination/treatment and/or at the time of discharge. If you have financial limitations, please let the staff know prior to treatment and, if possible, we will modify your pet’s treatment plan. We do not charge services.

Boarding Check-In - For all clients, to be completed each time an animal is boarded. Only one form is required per client, even if multiple animals are being kept.

Absent Owner Release - If for some reason you are unable to pick up your pet and wish to have a family member or friend pick them up for you, please fill this form out.

Drop Off Form - For pets that need to be treated when an owner needs to drop them off at the clinic while working or running other errands.

Consent to Treat - Form giving us authorization to treat your pet after it is left in our care.

Surgery Check-In - For all surgeries, please fill out any information you can at home and we will have a staff member finish going over the form with you when you arrive.

Dental Check-In - For all surgeries, please fill out any information you can at home and we will have a staff member finish going over the form with you when you arrive.

Chemo Treatment Consent - For the treatment of specific conditions needing chemotherapy.
